The Eye Stalk hat only stares at the person wearing it and not nearby players (or where the wearer is looking). Also it’ll randomly become detached with the eyeball moving the middle of the player’s head and the tentacle remaining up top.

Adding to this, in Minigolf the Eye Stalk is buggier. The eyeball itself does not scale with the rest of the item and always stares at one specific point in the map (possibly the origin point).
